Source: Rheumatology, BMJ
Area: News
The British Society for Rheumatology (BSR) and the British Health Professionals in Rheumatology (BHPR) have published guidelines on management of polymyalgia rheumatica, the most common inflammatory rheumatic disease in the elderly, and one of the biggest indications for long-term steroid therapy. These guidelines cover diagnosis, management and monitoring of disease activity, complications and relapse. An editorial in the BMJ points out that “the current guidelines simplify the approach to the diagnosis and management of patients presenting with polymyalgic symptoms, although they are not supported by good scientific evidence-most of the r…

Polymyalgia Rheumatica with Bilateral Subclavian Artery Stenosis

The overlap between polymyalgia rheumatica and giant cell arteritis has prompted physicians’ attention since the early 1970s. Polymyalgia rheumatica occurs in approximately 50% of patients with giant cell arteritis, and approximately 15% of patients with polymyalgia rheumatica develop giant cell arteritis. Some experts consider polymyalgia rheumatica and giant cell arteritis to be different manifestations of the same pathologic process. Although the association of occlusive vascular disease is well known in giant cell arteritis, it is not considered to be a typical manifestation of polymyalgia rheumatica. Expression and function of toll-like receptors in peripheral blood mononuclear cells of patients with polymyalgia rheumatica and giant cell arteritis

Conclusion
These data suggest activation of TLR7 during the active phase of PMR and GCA which resolves with complete disease remission. Whether this finding is the consequence of the marked inflammatory process in these disorders or activation by natural ligands remains to be explored. (Source: Annals of the Rheumatic Diseases)

Can the prognosis of polymyalgia rheumatica be predicted at disease onset? Results from a 5-year prospective study

Conclusion. A higher PV in PMR increases the risk of prolonged steroid therapy and late GCA. Female sex and particular HLA alleles may increase the risk of late GCA. Starting patients on >15 mg prednisolone is associated with a prolonged steroid duration. (Source: Rheumatology)
