BSR and BHPR guidelines for management of polymyalgia rheumatica

The British Society for Rheumatology (BSR) and the British Health Professionals in Rheumatology (BHPR) have published guidelines on management of polymyalgia rheumatica, the most common inflammatory rheumatic disease in the elderly, and one of the biggest indications for long-term steroid therapy. There are difficulties in diagnosis, with heterogeneity in presentation, response to steroids and disease course. These guidelines cover diagnosis, management and monitoring of disease activity, complications and relapse. Polymyalgia Rheumatica with Bilateral Subclavian Artery Stenosis
The overlap between polymyalgia rheumatica and giant cell arteritis has prompted physicians’ attention since the early 1970s. Polymyalgia rheumatica occurs in approximately 50% of patients with giant cell arteritis, and approximately 15% of patients with polymyalgia rheumatica develop giant cell arteritis. Some experts consider polymyalgia rheumatica and giant cell arteritis to be different manifestations of the same pathologic process. Although the association of occlusive vascular disease is well known in giant cell arteritis, it is not considered to be a typical manifestation of polymyalgia rheumatica.
