Treatment of Polymyalgia Rheumatica: A Systematic Review [Review Article]

Conclusions  The scarcity of randomized trials and the high level of heterogeneity of studies on PMR therapy do not allow firm conclusions to be drawn. However, PMR remission seems to be achieved with prednisone treatment at a dose of 15 mg/d in most patients, and reductions below 10 mg/d should preferably follow a tapering rate of less than 1 mg/mo. A systematic review of the evidence on treatment of polymyalgia rheumatica concludes that there is a scarcity of controlled trials and significant heterogeneity in those that exist; as a result, it is not possible to draw firm conclusions on the optimum treatment.

More Night Than Day – Circadian Rhythms in Polymyalgia Rheumatica and Ankylosing Spondylitis – REVIEW.

The circadian rhythm of symptoms in patients with chronic inflammatory diseases is well known. Circadian rhythms could be used to identify targets for time-adapted antiinflammatory therapies, which are administered prior to the flare of cytokine synthesis and inflammatory activity. In recent years, the diurnal variations in rheumatoid arthritis have been described precisely for pain, stiffness, and functional disability, as well as the underlying cyclic variations in hormone levels and cytokine concentrations. This review summarizes the current knowledge on circadian rhythms in other rheumatic diseases, focusing on polymyalgia rheumatica and ankylosing spondylitis.

Acute generalized exanthematous pustulosis induced by hydroxychloroquine: Three cases and a review of the literature.

Conclusions: This article reports 3 cases of AGEP related to administration of HCQ. HCQ-induced AGEP is a rare but severe, extensive, and acute reaction. No specific therapy is available, and correct diagnosis generally leads to spontaneous resolution once the causative drug has been withdrawn.
