A case of recurrent polymyalgia rheumatica-like complications with pregnancy
Abstract We encountered a 26-year-old Japanese woman with recurrent episodes of polymyalgia rheumatica-like symptoms associated with
pregnancy. At a 13-week pregnancy, she was admitted to our outpatient clinic, complaining of myalgia on both thighs, shoulders
and upper limbs. Laboratory examinations of blood yielded normal creatine kinase and mild elevation of erythrocyte sedimentation
rate. After administration of oral prednisolone (10 mg/day) was begun, her symptoms were gradually resolved by 1 week. Further
epidemiologic studies should be needed.

Myalgias most commonly occur in polymyalgia rheumatica (PMR). About 45% of patients with giant cell arteritis present with symptoms of PMR. Other vasculitides may also lead to arthralgia and myalgia. While shoulder and pelvic pain is characteristic for PMR pain often also occurs in the back of the neck and in the region of the thoracic spine. In addition, patients often present with malaise, morning stiffness and weight loss. CRP and ESR are elevated. Ultrasound and MRI delineate minor synovitis, tenosynovitis and bursitis in the shoulder. Hip joint synovitis and trochanteric bursitis are also commonly seen. PMR should be distinguished from rheumatoid arthritis. The initial treatment comprises a prednisolone dose of 15-25 mg/day, followed by a weekly decrease of 1-2…

Polymyalgia rheumatica and giant cell arteritis: An in‐depth look at diagnosis and treatment
Conclusions: Because of the varied and vague symptom presentations of PMR and GCA, primary healthcare providers often misdiagnose these pathologies, delaying treatment and relief of suffering. Textbooks descriptions often omit supplemental content necessary to understand the intricacies of the presenting symptoms, diagnostic tests needed for work‐up, and specific treatment for these overlapping pathologies.Implications for practice: Because PMR and GCA often present in the primary care setting, NPs must maintain a high index of suspicion when a patient over 60 years of age presents with vague complaints such as headache, weakness, anorexia, and malaise.
