Transient ischaemic attack in a patient with known temporal arteritis: a case report

Conclusions: Transient ischaemic attack is an alternative presentation or complication of an inflammatory disease such as temporal arteritis. The clinical implications of this case relate to the assessment of comorbid risk in TA and in tailoring the drug treatment. In using prednisolone treatments in such patients, general practitioners will need to carefully titrate drug doses and the duration of treatment to prevent complications. Clear evidence for the precise type of management remains to be established. (Source: Cases Journal)

Case report Dissecting aortic aneurysm of a patient with suspected polymyalgia rheumatica

A 62-year old patient was admitted to the rheumatology ward because of weakness, easy fatigue, fever, pain of the shoulder and pelvic girdle not improving after NSAIDs. Considering the patient’s examination and additional findings, polymyalgia rheumatica was suspected. The patient received corticosteroids, after which he felt better and a decrease of ESR was noticeable. Next the patient felt acute abdominal pain and since then he has suffered from constant abdominal pain and loss of weight. After some time blindness of the left eye and flaccid paresis of the left lower limb and the right upper limb appeared. In October, 2009, a 59-year-old man from the Balearic Islands, Spain, was referred to our department with a 10-month history of recurrent fever (up to 38·5°C), pronounced asthenia, arthralgia, and severe limb girdle muscle pain. He had had two episodes of pleuropericarditis in his youth, and had been diagnosed with polymyalgia rheumatica in 2007, which was treated with prednisolone; he was on 40 mg per day at the time of referral because of recurrent fever. Bilateral temporal artery biopsies done when his symptoms started were normal. (Source: LANCET)
